Health Families Polk is a voluntary, home visitation program designed to promote and enhance the healthy development of children and families by promoting positive parenting and reducing poor childhood outcomes including abuse and neglect. Outreach services are provided in order to build trusting relationships, promote positive parenting, enhance maternal-infant bonding and attachment, enhance the child's physical, emotional, and educational development, and enhance parent-child interaction. Assistance is also provided to participating families in the use of available community services and resources.

Healthy Families Facts

Healthy Families is credentialed by Prevent Child Abuse America. Since 1993, Healthy Families Polk has served over 1,000 families and almost 1,800 children throughout Polk County.

  • Less than 2% of the families served have been involved in confirmed reports of abuse or neglect.
  • Out of 1,790 children, 1,786 of them were not involved in a confirmed maltreatment report.
  • 95% of the children served are up to date on immunizations
  • 100% of the families enrolled in the Healthy Families program have been referred to community based services.
  • 94% of the families served had a medical provider.
  • 90% of the families served had a Family Support Plan developed within the first three months of program participation.
  • 95% of the families offered program services accepted enrollment.
