Please read carefully the following instructions on how to download and complete the Vendor Application.

STEP 1: Download Commodity Codes (60 KB) note: Print commodity codes list for reference or review, to determine all codes that apply to your business.

STEP 2: Download Vendor Application (306 KB) note: You may print the application and mail or fax it to us, or you may submit your application online.

STEP 3: Be sure to complete all information requested on the application including the following:

• Federal Employer I.D. Number (if you do not have one, please use your Social Security Number)
• Must check at least one option listed under the Minority Status/Other Status. If you check an option under the Minority Status, be sure and select yes/no under Certified W/MBE and Certified DBE.
• List the applicable Commodity Codes from our commodity code list in the space provided on the bottom of the application.
