1.  Fill Out Part A of the Application

Complete la application.

2. Ask Your Health Care Professional to Fill Out Part B.

Health Care Professionals include the following:

• Audiologist  (audiologó)
• Independent Living Specialist  (especialista de vida independiente)
• Licensed Clinical Social Worker (Clínico de Trabajo Social Licenciado)
• Licensed Mental Health Counselor  (Consejero Licenciado de Salud Mental)
• Occupational Therapist  (Terapista Ocupacional)
• Ophthalmologist  (Optamologó)
• Physician  (Medico)
• Psychologist  (Sicólogo)
• Registered Nurse (Enfermera Registrada)

Application Process - Proceso de Aplicación

Polk County Transit Services may contact the certifying Health Care Professional to verify the accuracy of the information provided. Within 21 days of receipt of the completed application, Polk County Transit Services will notify you by letter as to your eligibility status. If you have not received notification regarding eligibility within 21 days of submitting your application,  please call (863) 534-5500 or TDD (863) 534-3826. If a determination has not yet been made, you will be temporarily eligible until the determination has been made. If you are denied eligibility, you have the right to appeal the decision.

La División de Servicios de Transito de Polk puede contactar al profesional de salud para verificar la información provista.  Dentro de 21 días del recibo de la aplicación, la División de Transito le notificara por escrito acerca de su elegibilidad. Si usted no recibe notificación dentro de 21 días, llame al (863) 534-5500 o TDD (863) 534-3826. Si una determinación no se ha hecho, usted será determinado elegible temporáneamente hasta que una determinación sea hecha.  Si su elegibilidad fue negada, usted tiene el derecho de apelar la decisión.
