The Indigent Health Care Division (IHC) directs qualified uninsured or under-insured residents to contracted community partners who provide health and wellness services. The BoCC contracts with these physicians, nurse practitioners, dentists, health clinics and hospitals to allow participants, who otherwise would not be able to afford necessary care, to access and receive primary care, behavioral or mental health treatment, substance abuse treatment and dental services at little or no cost. The indigent healthcare sales surtax makes it possible for 36,563 people to receive healthcare through subsidizing the cost of care.
Community Partners Resource Packet
How do I know if I can utilize this program?
IHC assists participants with verifying their eligibility for appropriate healthcare programs and then connects them with providers who can give needed care. Community partners are available to those living at or below 200 percent of poverty. Those who do not qualify for the Polk Healthcare Plan, as well as Polk HealthCare Plan members who need specialty care outside of the healthcare plan can be connected to Community Partners.
Additionally, IHC aids participants in obtaining information, filling out and submitting program forms, and linking participants to other helpful resources.
If you have questions about what you qualify for or how to go about accessing our community partners call (863) 533-1111.
