Public Records Request Form
Under Florida law, you are not required to make a public records request in writing, or provide your name and reason for making a request. The online request form is provided for your convenience. If you do not wish to complete the online Public Records Request form, contact the Records Management Liaison Office. Please note: At times when public records requests require extensive use of information technology resources or extensive clerical or supervisory assistance, a special service fee may be charged in addition to the actual cost of duplication. Prior to the production of your public records request, you will be provided with an estimated cost. That cost must be paid first before the records request is completed. If costs are less than the estimate, the difference will be refunded.
